EasyRx Forms is a neat new enhancement feature for EasyRx lab accounts.   EasyRx Lab accounts can now activate EasyRx Forms to custom-create electronic forms or questionnaires to send to their practice accounts and get updates when those forms have been filled out. Examples include feedback regarding quality or a new practice intake questionnaire; really any form or question or signature capture needed.

All labs will see the "easyrx forms" tab on their dashboard. If you do not have EasyRx Forms activated yet, you will see the page below when you click on the tab:

Click Schedule Demo to set up a time to connect with our sales team to review and get this feature added to your EasyRx lab account.  



Using EasyRx Forms from the Dashboard

After you get EasyRx Forms added to your EasyRx lab account, when you click on the "easyrx forms" tab, you will see the page below:

Note: The EasyRx Forms page and dashboard are only accessible from your EasyRx account after you log in. If you get logged out of your EasyRx account you will also lose access to the EasyRx Forms Dashboard until you log back into EasyRx.

Click on EasyRx Forms Dashboard to go to this page, which will open a new tab in your browser:




From this dashboard, you can navigate to creating and managing your form templates, send forms to clients, track sent forms and responses, and view all the "Latest forms received" and "Pending forms".
To get back to the dashboard, click on the easyrxforms logo in the upper-left corner:






Manage Templates

Click on Manage Templates to start creating your forms so you can start sending them to your practice accounts:





Sharing of EasyRx Forms

Once you have completed creating Form Templates, you are now ready to share forms with practices.   You can share EasyRx Forms three ways:

1. When viewing a Rx in EasyRx
2. From the practice page in EasyRx
3. From the EasyRx Forms Dashboard:

1. Sharing EasyRx Forms from the View Rx Page

You can send a form right from the View Rx page. Click on Share EasyRx From toward the bottom left column, fill in the required fields, and SEND:


The email field will include available email addresses in EasyRx for the practice. 






2. Sharing EasyRx Forms when viewing a Practice

You can share EasyRx Forms when viewing a practice.  Lookup the practice, go to EasyRx Forms - share easyrx forms. This opens up the same Share EasyRx Forms form above. 



From this page, you can also track all of the forms you've sent to this particular practice:



3. Share EasyRx Forms from the Dashboard

Sending a form to a client:

What your clients see

After you send a form to a client, they will receive a similar email to the one below at the email address you entered.

The email message:



They'll click Open your Form, a new tab will open, and they will be prompted to input the password sent in their email:



Their form will look similar to this:

They can fill it out all at once, or Save and come back to it.  You can see the progress bar at the bottom.



Once they click Submit Form, they will see this message and they can close the tab:




And that's it!
